ADMISSIONS 2014-15

Applications are invited from candidates holding domicile of Punjab for admission in the Diploma in Cardiology (Duration 02 Years) essential pre-requisite are:


v) One year house job in recognized teaching institute including six months in General Medicine/ Cardiology signed and counter signed by the competent authority.


vi) Experience as Medical Officer/ Registrar in the Department of Cardiology for at least one year in a recognized teaching hospital duly signed and counter signed by the competent authority.


vii) Passed UHS Entry Test


viii) Appeared in aptitude interview.


The following are the guideline for the applicants to the Diploma in Cardiology:-


7- The candidate should apply to the Executive Director CPE Institute of Cardiology, Multan along with the original pay order bank draft of Rs. 1000/- in favour of the Executive Director. CPE Institute of Cardiology, Multan.


8- Complete application along with the original pay order bank draft of Rs. 1000/-. Can be submitted at CPE Institute of Cardiology, Multan / send to the Executive Director CPE Institute of Cardiology, Abdali Road, Multan through courier Service.


9- Government employees should submit their application through proper channel on downloaded forms for deputation to the Executive Director CPE Institute of Cardiology, Multan. The applicant will attach a recommendation letter/ N.O.C. from the Head of Institute/ Principal/ Chief Executive.


10- Others shall apply as private candidate on downloaded Admission Form for private candidates and send/ submit to the application forms directly to the office of the Executive Director, CPE Institute of Cardiology. Abdali Road, Multan.


11- Total number of seats 08


12- Attach attested photocopies of the following documents with the application.


I. One Passport size Photograph duly attested on front (Paste on admission form).


II. ID Card and Domicile.


III. Matriculation certificate, ESc. certificate, M.B.B.S MD degree (Equivalent certificate of PMDC should be attached).


IV. Valid Pakistan Medical and Dental Council (PMDC) registration certificate.


V. Attempt certificate from the Principal of the medical College stating the number of attempts and marks obtained in each professional examination of M.B.B.S. / M.D.


VI. House Job certificate with exact dates from the concerned Medical Superintendent as well as Professor/ Head of Department of the concerned Hospital (Both are necessary).


VII. Service certificate (relevant experience as M.O./ Registrar / Demonstrator etc.) from the concerned Medical Superintendent as well as Principal / Professor / Head of Department of the concerned Hospital Medical College.


VIII. First appointment letter from the Government of the Punjab, Health Department Lahore as Medical Officer on regular basis.


IX. Rural service certificate/ Army service certificate with exact dates and places where served from the concerned Director Health Services/ competent authority respectively.


X. Applications with incomplete documents will not be considered for admission.


XI. Admission is liable to be cancelled during the course if any discrepancy found in submitted documents.
